Coming in Fall 2026, a new medical office building carefully designed to enhance efficiency and convenience for patients and providers alike. The new building is located across the street from the hospital’s Elm Street campus in Claremont.

Unlock access to more services by establishing care with our team
Our Community Health Team is here to provide individualized assistance to patients of our primary care network for a variety of social needs including housing and food insecurity, counseling, medication assistance, and so much more! They aim to alleviate some of life’s biggest stressors so you can focus on your health and wellbeing. Patients who have established care at Associates in Medicine (AIM) or Valley Primary Care (VPC) are welcome to utilize the free support services of our Community Health Team.
