Patients who have established care with a Primary Care Provider at Associations in Medicine, Valley Primary Care or Valley Family Physicians are welcome to utilize the free services of our Community Health Team. To establish care and utilize the services listed below, please visit our Primary Care Page and find the provider that best fits your needs.
